Plan Description Anthem Medicare Preferred Core (PPO) is a Medicare Advantage Preferred Provider Organization plan with a Medicare contract.

This plan was designed to meet the health care needs of people on the go and those who expect more coverage, more freedom … yet still want to keep costs down. With this plan, you will enjoy:
• Out-of-pocket limits to keep you protected from high, unexpected medical costs
• Keeping the doctors you know and trust. You have the best of both worlds when the provider you choose is in your health plan’s Medicare Advantage PPO network.
• Access to all of the benefits that are available under standard Medicare coverage, including preventive coverage. In addition, we may offer expanded preventive services, prescription drug coverage, supplemental dental and vision coverage, wellness programs, and online tools
• Access to a basic gym membership at no additional cost

Just check out a few of the benefits:

• A large drug list (also known as a formulary), so you’re likely to find the prescriptions you need. And all the copayments and coinsurance amounts are listed in the Summary of Benefits, so you don’t have any surprises later.

Please keep this in mind: If eligible, you may only enroll in one Medicare prescription drug plan at a time, and you may not be enrolled in a Medicare Advantage Plan (HMO, PPO) at the same time, unless you are a member of a Medicare Private-Fee-For-Service plan or are enrolled in a 1876 Cost Plan which does not have Part D coverage built in. If you are enrolled in a Medicare Advantage HMO or PPO plan without prescription drug coverage, you'll be disenrolled if you join a Medicare Part D prescription drug plan.
